13th September 2010 - AMBULANCE chiefs have admitted they do not have a vital medical record relating to the death of Dr David Kelly.
South Central Ambulance NHS Trust has no copy of the Patient Report Form completed by paramedic Vanessa Hunt, who attended the scene of the former Government weapons inspector’s death near his home in Southmoor, near Abingdon, in 2003.
The form is considered one of the key documents required at an inquest. It gives medical information including pulse rates, blood pressure and skin condition.
Ambulance spokesman Gill Hodgetts said Oxfordshire Ambulance Service, which preceded SCAS, should have filed it as policy. {read rest}
